The early signs of pregnancy before a missed period can begin about a week after conception, once the embryo implants and the pregnancy hormone hCG starts to rise. The most common are breast tenderness, tiredness, light spotting, mild cramps, and passing urine more often. Each can also come before a period, so only a pregnancy test confirms pregnancy.
Pregnancy Symptoms Before Missed Period or PMS: Why They Feel the Same
Most pregnancy symptoms before a missed period are driven by progesterone, and progesterone rises after ovulation in every cycle, pregnant or not. That is why sore breasts, bloating, tiredness, and low moods turn up in both situations. The real difference comes a few days later. If an embryo implants, it releases hCG, which keeps the corpus luteum (the hormone-producing remnant of the follicle) active, so progesterone stays high and your period doesn't arrive. Without pregnancy, progesterone falls, and you bleed.

How Pregnancy Weeks Are Counted, and Why Week 1 Has No Symptoms
Doctors date a pregnancy from the first day of your last menstrual period, not from the day you conceived. In a 28-day cycle, week 1 is your period; ovulation happens around day 14, and fertilisation can only happen shortly after. So in "week 1" and most of "week 2" you are not yet pregnant.
That matters when you search for the first sign of pregnancy in week 1, or for pregnancy symptoms in the first week. There aren't any. Implantation and the first hCG happen in week 3 or early week 4, and your period is due at the end of week 4. In a longer or irregular cycle, every step shifts later. Our guide to physical signs that you are fertile explains how to spot ovulation.
Pregnancy Symptoms Week by Week: Week 1 to Week 4
Many online week-by-week symptom lists include symptoms that aren't hormonally possible at those stages. Here is what is actually happening each week in a 28-day cycle, and what you might reasonably notice. Think of it as a timeline rather than a day-by-day list of pregnancy symptoms.
Early pregnancy timeline: what happens and what you may notice, Week 1 to Week 4
Weeks are counted from the first day of your last period. A test can only turn positive after implantation and hCG has had time to build.
Week 1
Your period
Period symptoms only
No, you are not pregnant yet
Week 2
Follicle grows; ovulation around day 14; fertilisation within a day of ovulation
Fertile cervical mucus, mild ovulation ache
No
Week 3
Embryo travels to the womb and implants 6 to 12 days after ovulation
Usually nothing; sometimes light spotting or twinges
No, or only very rarely by blood test late in the week
Week 4
hCG rises; progesterone stays high
Possible sore breasts, tiredness, frequent urination, metallic taste
Sensitive urine tests may turn positive late in the week; blood tests earlier
Week 1 and Week 2: before conception
The first sign of pregnancy in week 1 is simply your period. Week 1 and week 2 look the same whether a cycle ends in pregnancy or not. If you are trying, this is the time to track ovulation, for example with ovulation test kits, and to keep taking folic acid.
3 Weeks Pregnant Symptoms: Fertilisation and Implantation
After fertilisation, the embryo travels down the fallopian tube for several days. It implants in the womb lining 6 to 12 days after ovulation, most often on days 8 to 10, according to a landmark study of 221 women trying to conceive [1]. Some women notice a little spotting or a twinge, like pulling, around this time. Many notice nothing at all, which is completely normal.
No hormonal symptom can start before implantation because the body only makes hCG once the embryo attaches. If you are going through IVF, the timing is different; see our day-by-day guide to implantation after IVF.
4 Weeks Pregnant Symptoms: The Days Before Your Period Is Due
This is where symptoms, if they come, usually begin. hCG keeps the corpus luteum working, so progesterone stays high instead of falling. Breasts may feel fuller or tingly. You may feel unusually tired by mid-afternoon, need to pass urine more often, or notice a metallic taste or a new dislike of tea or coffee [9, 10].
Even so, many women still feel exactly as they do before a period. In one study, only about half of women with ongoing pregnancies had noticed any symptom by day 36 from their last period, roughly a week after the missed period.
What does early pregnancy feel like?
For most women, early pregnancy feels like a slightly stronger version of their usual premenstrual week: heavy breasts, a low-grade tiredness, a bloated tummy and a mild ache low in the pelvis. The difference tends to become clearer over time. Instead of easing once a period starts, these feelings carry on, and nausea may join them around or soon after the missed period. These feelings alone can't confirm pregnancy; a test on the right day can.
10 Earliest Signs of Pregnancy Before a Missed Period
These are the symptoms of pregnancy before a missed period that women most often report, in the days before a period is due. None of them on its own proves pregnancy.
Breast tenderness: Fuller, sore, or tingling breasts.
Tiredness: A heavier fatigue than usual, linked to rising progesterone.
Light spotting: A few pink or brown spots, lighter than a period and lasting a day or two.
Mild cramps: Period-like twinges low in the tummy, without a period following.
Frequent urination: Needing to pass urine more often, including at night.
Bloating and constipation: Progesterone slows the gut, so you may feel full or blocked.
Metallic taste or food aversions: A sudden dislike of tea, coffee, or fried food.
Sensitivity to smells: Cooking smells or perfume may suddenly feel too strong.
Increased discharge: thin, milky white discharge without itching or odour.
Mood changes: feeling tearful or irritable, much as you might before a period.

How Do I Tell if I'm Pregnant Before My Missed Period?
You can only tell with an hCG test, because symptoms before a missed period overlap with PMS. A home urine test may turn positive a day or two before your period is due [8], but an early negative means little. Test from the first day of your missed period, or ask for a blood beta hCG test.
Home urine pregnancy test
Home tests detect hCG in urine and are reliable from the first day of a missed period [8]. Even then, about 10% of pregnancies are not yet detectable that day because implantation happened late; a week later, about 3% [2]. If you don't know when your period is due, test at least 21 days after unprotected sex [8]. If the test is negative and your period still has not come, repeat it after 48 to 72 hours.
Blood beta hCG test and early scan
A quantitative serum beta hCG test measures the exact level of hCG in your blood and can pick up a pregnancy earlier than a urine test. A single value tells you that hCG is present; two values 48 hours apart show whether it is rising as expected, which is useful after IVF, after a previous loss or when there is pain or bleeding. An ultrasound scan cannot see a pregnancy before a missed period, because the pregnancy is still far too small to show; early scans are usually booked a couple of weeks after a positive test.
When to test: choosing the right test for your situation
Testing too early is the most common reason for a confusing result. Match the test to your situation.
Regular cycle, period due
Home urine test
From the first day of the missed period
Irregular cycle or unsure of dates
Home urine test
At least 21 days after unprotected sex
Negative test, still no period
Repeat urine test, then see a doctor
After 48 to 72 hours
After IUI or IVF
Serum beta hCG
On the date your fertility team gives you
Pain or bleeding with a positive test
Serum beta hCG and scan
Same day, at a clinic or emergency department
A note for IUI and IVF patients
Progesterone support after IUI or embryo transfer causes sore breasts, bloating and tiredness whether or not treatment has worked, so symptoms in the two-week wait mean even less. If you used an hCG trigger injection, a home test taken too soon can also pick up the injection rather than a pregnancy. Wait for the blood test your team schedules; our guide to early pregnancy care after IVF explains what happens after a positive result.
How Much Do Early Pregnancy Tests Cost in India?
Early pregnancy testing in India ranges from a home urine kit to a quantitative serum beta hCG blood test and, after a positive result, an early transvaginal ultrasound. Your total depends on your city, the centre, how many blood tests you need, and whether you add a consultation in the same visit.

Symptoms That Need Medical Attention
Most early symptoms are harmless, but a few need prompt care, especially after a positive test. About 1 in 90 pregnancies is ectopic, growing outside the womb, usually in a fallopian tube [6]. Symptoms of an ectopic pregnancy usually appear around week 6, about two weeks after a missed period, but can come sooner [6].
Warning signs in early pregnancy and what to do
Each row pairs a symptom with what it may mean. These are reasons to be seen the same day, not to wait and watch.
Having no symptoms before a missed period is the norm, not a warning sign. In a prospective study that followed women from before conception, about 1 in 10 with ongoing pregnancies still had no symptoms at all by the end of week 8. Symptoms also started later in women who smoked. hCG and progesterone rise at different speeds in different women, and sensitivity to them varies just as much. In the same study, symptoms were less common in pregnancies that ended very early, but many healthy pregnancies have few symptoms, so their absence alone tells you nothing about your own pregnancy.
Realistic expectations: how many women notice symptoms, and when
These figures come from studies that tracked women daily from before conception, so they reflect real timing rather than memory.

FAQs
Your questions, clearly answered
Find clear, trusted answers to the most common questions about IVF—designed to guide and support you every step of the way.
What happens 5 days before your period if you're pregnant?
Five days before your period is due, implantation has usually just happened, and hCG is only beginning to rise. Most women feel nothing different from a normal premenstrual week. Some notice sore breasts, tiredness or light spotting. Most home tests are still negative at this point, so testing this early often gives a false reassurance or a false worry.
What is the first thing you notice when you get pregnant?
For women with regular cycles, the first reliable sign is a missed period. Before that, the earliest changes people report are usually breast tenderness and unusual tiredness, both caused by progesterone. Nausea typically arrives a little later, around or just after the missed period. Because these overlap with PMS, a pregnancy test is the only way to know.
How does your stomach feel when you're early in pregnancy?
Many women describe mild, period-like cramps or a pulling feeling low in the tummy, along with bloating and a full feeling. Progesterone slows digestion, so constipation and wind are common. Mild cramps without bleeding are usually normal. Severe pain, pain on one side or pain with bleeding needs a same-day medical check.
Can I do a pregnancy test before my missed period?
A sensitive home test may turn positive a day or two before your period is due, but a negative result that early is unreliable. About 1 in 10 pregnancies is not detectable even on the first day of a missed period. For a clear answer, test on or after the missed day, or ask for a blood beta hCG test.
Is back pain a sign of pregnancy before a missed period?
Lower back ache can happen in early pregnancy, but it is just as common before a period, so it is not a useful sign on its own. Back pain with fever, burning urine or one-sided tummy pain needs medical attention. On its own, it cannot tell you if you are pregnant.
Can stress cause symptoms that feel like pregnancy?
Yes. Stress can delay ovulation, which pushes your period later, and it can cause tiredness, nausea, appetite changes and poor sleep. Waiting to find out if you are pregnant is stressful in itself, which can make you notice every twinge. A test taken on the right day is more reliable than relying on your body.
What if my test is negative but I still feel pregnant?
You may have tested too early or ovulated later than you thought. Wait 48 to 72 hours, then test again with a fresh kit and follow the instructions exactly. If the test is still negative and your period hasn't come within a week, see a doctor, who can arrange a blood beta hCG test and check for other causes.
What does early pregnancy discharge look like before a missed period?
Early pregnancy discharge is usually thin, milky white, and mildly scented, and there may simply be more than usual. On its own, it cannot confirm pregnancy, because discharge also changes through every cycle. Discharge that itches, burns, smells strongly or is green or grey suggests an infection and should be checked.
