High Prolactin Levels and Fertility: Why Hyperprolactinaemia Can Stop You Ovulating

High prolactin levels, or hyperprolactinaemia, mean the pituitary gland is releasing more of the milk hormone prolactin than it should. In women who are not pregnant or breastfeeding, levels above about 25 ng/mL are usually considered high (WHO, 2025). It can quietly switch off ovulation, and in most women it can be treated.

‍

A normal prolactin level

‍

What is a Normal Prolactin Level for a Woman?

A normal prolactin level for a non-pregnant woman is below about 25 ng/mL, and below about 20 ng/mL for a man (WHO, 2025). Prolactin rises naturally during pregnancy and breastfeeding, so those readings are interpreted differently. Labs in India report in ng/mL or mIU/L; 1 ng/mL is roughly 21 mIU/L (Endocrine Society, 2011).

Prolactin ranges and what they usually mean

Use this as a guide to your report, not a diagnosis. Your lab's own reference range comes first, and one raised reading always needs a repeat.

‍

Below 25 (women)

Normal for a woman who is not pregnant

Prolactin is not the reason for missed ovulation

25 to 30

Borderline

Repeat the test calmly in the morning

30 to 60

Mildly raised (AOGD, 2017)

Often stress, a medicine or macroprolactin; repeat before treating

60 to 100

Moderately raised

Periods often become irregular; look for a cause

Above 100

Severely raised

Periods often stop; a pituitary scan is usually advised

Above 250

Very high

Usually a prolactinoma (Endocrine Society, 2011)

What Happens When Prolactin Levels are High?

When prolactin levels are high in a woman who is not breastfeeding, the brain turns down the signals that make the ovaries release an egg. Periods become irregular or stop, ovulation becomes unreliable, and getting pregnant takes longer. The WHO data put raised prolactin behind about 6.7% of female infertility (WHO, 2025).

Most women first encounter the term "high prolactin" on a blood test report, often after experiencing irregular menstrual cycles. Some may notice a milky nipple discharge, while others have no symptoms and only discover the condition during a fertility evaluation.

The good news is that this is one of the more treatable causes of not ovulating. Indian Fertility Society guidance places raised prolactin in 10 to 25% of women whose periods have stopped and in about 30% of women seen for galactorrhoea or infertility, quoting the published literature (IFS, 2019). That is why India's national treatment guideline asks doctors to check prolactin and TSH in every woman with irregular periods or infertility (MoHFW STG).

How High Prolactin Stops Ovulation

Prolactin doesn't act directly on the ovary. It works upstream, in the brain, which is why the fix is usually a tablet and not surgery.

The hormone chain behind it

Your hypothalamus sends pulses of a hormone called GnRH to the pituitary. Those pulses tell the pituitary to release FSH and LH, the two hormones that stimulate follicle growth and trigger ovulation. High prolactin slows those GnRH pulses (Endotext). FSH and LH drop, the follicle stalls, and no egg is released that month.

Estrogen also falls when this goes on for months. That explains the vaginal dryness some women notice, and why doctors care about bone health in long-standing cases.

Hyperprolactinaemia symptoms in women

The common high prolactin symptoms in women are:

Irregular or missed periods. Cycles stretch beyond 35 days or stop altogether.

Milky nipple discharge (galactorrhoea). Only about one in three women with high prolactin have it, so its absence proves nothing (ASRM, 2024).

Difficulty getting pregnant. This is usually because ovulation has become rare, sometimes for months at a stretch.

Low sex drive and vaginal dryness. Both follow the fall in estrogen.

Headaches or blurred side vision. Rare, and a sign that a larger pituitary growth is pressing nearby. These need prompt review.

How it shows up in men

Men can have high prolactin too. It tends to lower testosterone, so the signs are low libido, erection problems and sometimes a lower sperm count. A semen analysis and hormone check usually sit alongside the prolactin test when a couple is being assessed together.

Mood, sleep and bones

Women with long-standing high prolactin sometimes describe low mood, poor sleep or tiredness. Low estrogen over years can also thin the bones. Both usually improve once prolactin returns to range, which is another reason not to leave a confirmed high result untreated.

‍

The causes of high prolactin in women

‍

What Causes High Prolactin in Females?

The causes of high prolactin in women, roughly in the order a specialist checks them, are:

Medicines. The most common cause in Indian practice. Anti-nausea tablets such as domperidone and metoclopramide, many antipsychotics, some antidepressants, verapamil, and opioid painkillers can all raise it. Typical antipsychotics raise prolactin in 40 to 90% of people who take them (Endocrine Society, 2011). Levels usually settle within 3 to 4 days of stopping the culprit (Pituitary Society).

Underactive thyroid. Many infertile women with hypothyroidism also have raised prolactin, and correcting the thyroid often corrects both (ITS and FOGSI).

A prolactinoma. A benign pituitary gland growth. It is not cancer. Most are small, under 1 cm, and respond well to tablets (Endocrine Society, 2011).

PCOS. A mild rise is common in PCOS and should not be taken as a sign of a prolactinoma (MoHFW STG).

Stress and the blood draw itself. Physical stress, a painful needle, strenuous exercise, or breast stimulation can raise prolactin two to four times for under an hour (Pituitary Society consensus, 2023).

Kidney or liver disease. The body clears prolactin more slowly.

Chest-wall injury or surgery. Less common, but on the list (Endocrine Society).

Macroprolactin. A large, mostly inactive form of prolactin that the test still picks up. It is found in about one in five high readings (ASRM, 2024) and usually needs no treatment.

Sometimes no cause is found. This is called idiopathic hyperprolactinaemia, and it is treated the same way.

How is High Prolactin Diagnosed and Treated?

Doctors diagnose high prolactin with a simple blood test, repeated if the first result is only mildly raised, followed by tests to find the cause. Treatment targets the cause: stopping or switching a medicine, correcting the thyroid, or using a dopamine agonist tablet, with cabergoline as the preferred first choice (NICE NG257).

Getting the test right, and the work-up

A surprising number of "high" results in clinic come from the way the sample was taken. These steps help:

Book a morning slot. Two to three hours after waking is ideal (IFS, 2019).

Avoid triggers the day before. No breast examination, nipple stimulation, or hard workout.

Sit quietly for a few minutes first. A single sample is enough if it is taken without undue stress (Endocrine Society, 2011).

Repeat a mild rise. If the value is under five times the upper limit, the consensus is to recheck (Pituitary Society consensus, 2023).

If the rise is confirmed, your doctor will usually order a TSH test, a pregnancy test, a review of all medications you take (including over-the-counter acidity and nausea tablets), and a macroprolactin test. A pituitary MRI is kept for women whose prolactin stays high without another explanation, or who have headaches or vision symptoms.

Treatment options, and the fastest way to bring prolactin down

The fastest way to reduce prolactin depends on why it is high:

Medicine-induced. When it is safe, the doctor who prescribed the medicine switches you to one that doesn't raise prolactin. Levels usually settle within days, but never stop a psychiatric medicine yourself.

Thyroid. Thyroid replacement often brings prolactin down within weeks.

Prolactinoma or no clear cause. Cabergoline is the first choice. WHO and NICE both now prefer it to bromocriptine (WHO, 2026; NICE NG257). In trials, it normalised prolactin in 95% of women with small tumours (Endocrine Society, 2011). It is taken once or twice a week, so it fits easily into daily life.

Bromocriptine. An older alternative, sometimes chosen by doctors in specific situations.

Surgery. Rarely needed, and only when tablets fail or are not tolerated.

Macroprolactin only. Usually no treatment at all.

Diet changes and herbal remedies do not lower a confirmed high prolactin level. Sleep, stress care, and a healthy weight still help your fertility overall.

When should you see a fertility specialist?

See a fertility specialist if your prolactin report is high and you are trying to conceive, or if your periods have changed and you do not know why. Ovulation problems justify earlier review than the usual one-year rule. The table below shows how quickly to act.

When to book, and how soon

Match your situation to the timing. If you are unsure, book the earlier slot.

Can You Get Pregnant With High Prolactin?

Yes, you can get pregnant with high prolactin once it is treated, and many women do so without IVF. When prolactin falls, ovulation usually returns on its own. In a large trial, 72% of women on cabergoline ovulated or conceived, compared with 52% on bromocriptine (Endotext). Pregnancy can even happen before periods settle (Pituitary Society consensus, 2023).

If ovulation still does not return after prolactin is normal, your doctor will look for a second cause, such as PCOS or low ovarian reserve, and may suggest ovulation induction with cycle monitoring. A follicular study shows whether an egg is growing and releasing. If you also have PCOS, our guide to PCOS and fertility explains how the two are managed together.

FAQs

Your questions, clearly answered

Find clear, trusted answers to the most common questions about IVF—designed to guide and support you every step of the way.

Is 42 a high prolactin level?

Yes, 42 ng/mL is mildly raised for a non-pregnant woman, since the usual upper limit is about 25 ng/mL. This result is often caused by stress, a recent meal or exercise, medication, or macroprolactin. Your doctor will usually repeat the test in the morning before deciding on treatment or scans.

Is high prolactin a serious issue?

High prolactin is rarely dangerous, but it is worth treating. The most common causes are medicines, thyroid problems and small benign pituitary growths, which are not cancer. Left alone, it can stop ovulation and, over years, thin the bones. Sudden severe headache or changes in side vision need urgent medical review the same day.

Is prolactin high in PCOS?

Prolactin can be mildly raised in some women with PCOS, but PCOS does not usually cause high levels. India's national treatment guideline advises that a mild rise in PCOS should not be read as a prolactinoma. If your prolactin is clearly high, your doctor will look for another cause, such as a medicine, thyroid problem or pituitary growth.

Is it difficult to conceive if prolactin is high?

It can be harder while prolactin is high, because ovulation becomes irregular or stops. Once prolactin is brought down, most women start ovulating again and can conceive naturally. Trials show cabergoline restores ovulation or pregnancy in about 72% of women. If it does not, ovulation induction or a check for other fertility factors is the next step.

Should I stop cabergoline when I find out I am pregnant?

Most women with small pituitary growths or no clear cause are advised to stop cabergoline as soon as pregnancy is confirmed. The Endocrine Society guideline recommends this because small growths rarely enlarge in pregnancy. Do not stop it on your own. Call your specialist the day you get a positive test, because larger growths need a different plan and closer watching.

Can stress cause high prolactin?

Yes. Physical and emotional stress can raise prolactin, though usually only a little and only for a short while. Even the stress of a painful blood draw can push it up two to four times for under an hour. That is why doctors always repeat a mildly raised result.

What foods should you avoid in hyperprolactinaemia?

No food has been shown to cause or cure high prolactin, so there is no list of banned foods. What matters more is your medicine list. Some common tablets for acidity, nausea and mood raise prolactin. A balanced Indian diet with dal, vegetables, curd, and whole grains supports fertility overall while treatment does its work.

Can high prolactin cause weight gain?

The evidence that prolactin itself causes weight gain is limited. Weight gain is more often driven by the conditions that sit alongside it, such as hypothyroidism or PCOS, or by the medicines that raised prolactin in the first place. Treating the cause usually helps both prolactin and weight. If your prolactin report is high or your periods have changed, you do not have to work it out alone. A Cloudnine Fertility specialist can confirm the result, identify the cause, and plan treatment to protect your chances of pregnancy. Book a consultation with a Cloudnine Fertility specialist and bring your reports.
